From 1st July 2021 NHS England are updating their main privacy/data sharing policy, along with the way patients can opt in/out of sharing their data within the NHS.

Please note that patients that have opted out of data sharing in the past have already got the relevant information (code) in their records. 

However if you are unsure, or wish to opt out of the NHS England data sharing policy please follow the above link, complete the provided opt-out form, and return this form to the practice.
